HOW TO REQUEST FOR THE USE OF PLAZAS

Description: This service is offered to person and/or organization who wish to use public parks for their event/activity.

1  Client/s: Any person/establishment who wish to use public parks/plazas

3  Location: Parks and Playgrounds Commission, Plaza Independencia, Cebu City
4  Availability: Monday-Friday / 8:00 AM - 5:00 PM
5  Duration of the Service: 1 day and 43 minutes
6  Requirement/s:
                             a) Letter request addressed to: Hon. Ma. Nida C. Cabrera, Chairman of the Committee on Parks and Playground, Wildlife, Ecology and Environmental Management
                             b) Permit to Use (available at the Parks office)

7  Procedure in Availing the Service:

1. Submit request letter to Parks and Playground Commission office Receives the letter from the client Janet G. Libarios 2 min.
2. Coordinate with staff as to availability of the date and venue of the activity Checks the availability of the date and venue for the activity 5 min.
3. Wait for the approval of the Parks and Playground Commission for the use of the park. Seeks approval from the Chairman of the Parks and Playground Commission.

Note: No function is allowed in Fuente Osmeña except during Sinulog and Pasko sa Sugbo.

4. Make donation to the Parks and Playground Foundation for the use of the park. 4.1. Accepts donation in behalf of the Parks and Playground Foundation. Janet G. Libarios 5 min.
4.2. Issues temporary receipt 5 min.
4.3. Endorses donation to the foundation the following day
4.4. Issues Official Receipt for the donation received 10 min.
4.5. Hands over receipt to the client 1 min.
5. Submit Permit to Use to Parks & Playgrounds Commission Receives signed Permit to Use form from client Janet G. Libarios 5 min.

HOW TO BE A PART OF A MASS WEDDING

Description: This service legalizes and solemnizes the lives of couples without legal impediments to marry.

1  Client/s: Cebu City Constituents – couples who have lived together for at least 5 years

3  Location: KALIPI Federation Office
                     2nd floor, Ramos Public Market
4  Availability: Registration - Monday-Friday / 8:00 AM - 5:00 PM
                         Mass Wedding - every September
5  Duration of the Service: 2 months, 1 hour & 15 minutes
6  Requirement/s:
                               a) Birth Certificate - 1 copy original & 2 copies photocopy
                               b) Certificate of Attendance – Pre Marriage Counseling (CHD- HRD) – 1 Copy original & 1 photocopy
                               c) Community Tax Certificate (CEDULA) – 1 original & 2 photocopies
                               d) Barangay Certificate
                               e) Certificate of No Marriage (CENOMAR) issued by NSO – 1 original and 1 photocopy
                               f) For Ages:
                                       18-20 – Appearance for both parents; consent
                                       21-24 – Appearance of both or either parent or parents' advice
                               g) Pre- cana Seminar (by Church)
                               h) Confiramation Certificate
                               i) “Tawag sa Simbahan”
                               j) Permit to Marry from Parish
                               k) Baptismal Certificate
