HOW TO GET A HEALTH CARD FOR NIGHT ESTABLISHMENT ENTERTAINERS

Description: Clients’ Health cards are updated weekly after STI screening and treatment, if found positive for STI.

  Client/s: Night/recreational establishment entertainers, masseuses/masseurs working in Cebu City

  Location: STD/ AIDS Detection Unit, Left wing
                       City Health Department (CHD)
  Availability: Monday-Friday / 8:00 AM - 5:00 PM
  Duration of the Service: 2 hours & 33 minutes
  Requirement/s: a) 3 copies of recent 2x2 picture (colored or black and white);
                                b) Chest X-ray result with big film;
                                c) Residence certificate;
                                d) Birth Certificate (for 18-25 years old)
  Fee/s: Php 25.00 (Php 10.00 for Health Card and Php 15.00 for Gram staining)

  Procedure in Availing the Service:

Follow these steps...	Wait while Person-in-charge...	Approach...	It will take...
1. Submit requirements to clerk.	1.1. Checks for completeness of documents 1.2. Types in pertinent data in health card & gives it to the patient	Clerk	8 min.
2. Proceed to admitting section and hands in health card & wait for the no. to be called	Receives filled up health card and gives to client glass slides w/ nos.	Clerk	12 min.
3. Proceed to examining room as patients number is called.	Does pelvic exam and collects specimen for gram staining	PHM/PHN	3 min.
4. Wait for laboratory official result	Submits specimen to the laboratory for examination laboratory hands in to STD section the official results. Doctor interprets lab results. Logs in positive results in log book. Patient is called for treatment.	Nurse attendant	2 hours
5. Proceed to the doctor’s room.	Prescribes and gives health education to client. Informs client of follow up visit .Gives to client health card.	MO	10 min.
