HOW TO AVAIL OF CONTINUITY OUTPATIENT CLINIC AT CCMC
Description: Provision of follow-up care to patients who initially sought consultation at the OPD/ER/Eye and Dental Clinic and return for follow-up visit or were previously admitted and discharged from the hospital and were instructed to go to the OPD for follow-up visit.
Client/s: Patients who have had previous consultations or admissions with instructions for follow-up care.

Availability: Monday – Friday, 8:00 AM- 5: 00 PM
Duration of the Service: 5 minutes
Requirement/s: Referral slip for follow-up care from attending physician for discharged patients (for previously admitted), hospital issued patient’s identification card
Procedure in Availing the Service:| A. For patients with hospital identification card or case number already: Present your hospital issued identification card and/or referral slip issued by attending physician to the registration section of the respective service area (OPD, Eye Clinic, Dental Clinic) |
A.1. Registers the patient’s name in the patient’s registry or logbook. A.2. Puts the patient’s hospital identification card in the box which will be used by the clerk to retrieve his/her chart. A.3. Retrieves the patient’s chart. |
