| 2. Submit for vital signs. | Gets the weight, pulse rate & B/P records in the FP Form 1 | BHW | 3 min. |
| 3. Proceed to admission area for registration and examination. | Counsels, examines & records in the FP Form 1 | MO, PHN, PHM | 7 min. |
| 4. Choose method after proper assessment. | Provides the method chosen after assessment | MO, PHN, PHM | 10 min. |
