HOW TO SECURE TAX DECLARATION

Description: Issuance of tax declaration for newly declared properties or for transfer of ownership.

1  Client/s: Property Owners or their representative
2  Contact Person/s: Wilhelma Borgonia

3  Location: City Assessor’s Office
                      4th Floor, Executive Bldg., Cebu City Hall
4  Availability: Monday-Friday / 8:00 AM - 5:00 PM
5  Duration of the Service: 6 working days
6  Requirement/s:
                                 For properties declared for the first time
                                1. Completely filled out Real Property Assessment Request Form
                                2. Certificate of Occupancy or Building Permit
                                3. Tax Declaration of the land
                                4. Latest Tax Clearance Certificate of the land
                                5. Certificate of house ownership form the Brgy.
                                6. Affidavit of legal heirs with lot consent (if dead)

                                For Transfer of Ownership
                                1. Completely filled out Real Property Assessment Request Form
                                2. Certification Authorizing Registration (CAR) from BIR
                                3. Deed of Sale / Certification of sale/Affidavit of consolidation
                                4. Transfer Certificate of Title/Original Certificate of Title
                                5. Transfer Tax receipt from City Treasurer’s Office
                                6. Tax declaration of the land and latest Tax clearance Certificate of land

7  Procedure in Availing the Service:

1. Submit requirements Receives and checks the completeness of supporting documents, records the transaction in the logbook and forwards the transaction to the DIC. Wilhelma C. Bogonia

2. Arrange for date and time of inspection. District-in-charge DIC) and Tax Mapper
3. Guide the Tax Mapper and DIC to the inspection site. DIC schedules inspection. 45 minutes depending on location
DIC and TM conducts inspection.
DIC prepares field appraisal assessment sheet(FAAS), then forward FAAS to Tax Mapper.
Tax Mapper updates Tax Mapping control roll, then forwards FAAS to GIS Jeannie Ann P. Soco
or Juanita V. Valdez
(Releasing Clerks)
3-5 working days
GIS clerk updates record
Computer section encodes data and prints TD 5 min.
TD is submitted to signatories (DIC, Tax Mapping Head and City Assessor.
